Proclaim Care has an excellent opportunity for experienced Rehabilitation Case Managers to work with us on an associate basis. Part of Proclaim Group, we are a national, award-winning provider of rehabilitation solutions for individuals who have been injured and/or are absent from work. We are seeking associates based throughout the UK.

Summary of Essential Requirements:

  • Right to work in and current resident of the UK
  • Qualification in Occupational Therapy, Nursing or Physiotherapy
  • Active registration with the HCPC or NMC
  • Valid driving licence

Job Description:

This is an exciting opportunity to support service users with complex injuries and health conditions to optimise their recovery and reach their rehabilitation and return to work goals. Working within a commercial (personal injury and medical negligence) context, this field-based role involves navigating service users through their personal rehabilitation journey by:

  • Conducting in-person and virtual assessments to determine service users’ physical, psychological and social needs on an ongoing basis
  • Devising and coordinating goal-focussed rehabilitation programmes, which includes a combination of both private and statutory services
  • Collaborating with family members and other inter-disciplinary professionals
  • Communicating regularly with external stakeholders e.g. solicitors, insurers and employers
  • Writing comprehensive reports for the instructing parties to provide updates, demonstrate progress and seek funding approval
  • Facilitating return to work and/or education

Person Specification:

We are looking for experienced Rehabilitation Case Managers with a qualification in Occupational Therapy, Nursing or Physiotherapy and active HCPC or NMC registration. A proven clinical background in complex orthopaedic trauma is required.

Experience of, or interest in, working with the following clinical specialisms would be of benefit, but is not essential:

  • Acquired brain injury
  • Spinal cord injury
  • Amputations
  • Paediatrics

Rehabilitation Case Managers work from home but regional travel to meet with service users, treatment providers and employers is a large part of the role. A valid driving licence is therefore essential.

To excel in this role, you should demonstrate:

  • Strong clinical reasoning skills with a solutions-focussed approach
  • Effective communication, report writing style and interpersonal skills
  • Excellent organisational and administrative skills with attention to detail
  • Confidence with utilising IT software packages
  • The ability to work independently and as part of a multidisciplinary team
