Are you passionate about patient safety, quality improvement and helping services learn and improve? We're looking for a Clinical Governance & Patient Safety Facilitator to join our friendly and supportive Quality Team at Ashgate Hospice. This is an exciting opportunity to support the delivery of our clinical governance, patient safety and clinical effectiveness agenda, helping to ensure we continue to provide safe, effective and compassionate care.
Working closely with the Head of Clinical Governance & Patient Safety, you'll coordinate a range of governance activities including incident management, risk management, medicines governance, clinical audit, quality improvement and organisational learning. You'll work collaboratively with teams across the Hospice to identify opportunities for improvement, monitor progress and help embed a culture of openness, learning and continuous improvement. This is a varied and rewarding role where you'll see the direct impact of your work on patient care, staff development and service improvement.
Main duties of the job
- Support the delivery of the Hospice's clinical governance, patient safety and quality improvement agenda.
- Coordinate governance processes including incident management, investigations, risk management, medicines governance and regulatory compliance.
- Support clinical audit, benchmarking and service improvement activities to promote safe, effective and evidence-based care.
- Monitor actions arising from incidents, audits, complaints and reviews, ensuring improvements are implemented and sustained.
- Work collaboratively with clinical teams to promote a culture of openness, learning and continuous improvement.
- Contribute to governance reporting, meetings, training and organisational learning activities.
- Support patient experience initiatives and deputise for the Head of Clinical Governance & Patient Safety when required.

Job responsibilities
We're looking for an organised and proactive registered healthcare professional with experience in clinical governance, patient safety, quality improvement, audit or risk management. You'll have strong communication, analytical and organisational skills, with the ability to build effective working relationships across multidisciplinary teams. You'll be confident coordinating governance activities, supporting investigations and using information to identify learning and drive improvement. We're seeking someone who is passionate about improving patient care, supporting a positive learning culture and helping services continuously improve. In return, you'll join a supportive and collaborative team, benefit from ongoing professional development opportunities and play an important role in strengthening quality, safety and patient experience across the Hospice.
Qualifications
- Registered Health Professional (NMC/GMC/HCPC/GPhC) with relevant 1st degree (or equivalent experience).
- Evidence of continuing professional development (CPD).
- Training in incident investigation methodologies, or equivalent knowledge and experience.
- Training or experience in clinical governance and/or patient safety.
- Postgraduate qualification, or working towards one, in Patient Safety, Quality Improvement, Leadership, Palliative Care or a related subject.
- Teaching, assessor or facilitation qualification, or equivalent experience.
- Additional training in communication, counselling or facilitation skills.
Experience
- Experience of clinical governance, patient safety, clinical audit, quality improvement or risk management activities.
- Knowledge of clinical governance frameworks, patient safety principles and regulatory requirements, including CQC standards.
- Experience of supporting incident reviews or investigations and promoting organisational learning.
- Experience of using data, audit findings and governance information to support service improvement.
- Strong organisational, analytical and problem‑solving skills, with the ability to manage competing priorities.
- Excellent communication, influencing and relationship‑building skills, with the ability to work effectively across multidisciplinary teams.
- Experience of report writing and presenting information clearly to a range of audiences.
- Proficient IT skills, including Microsoft Office and electronic governance or risk management systems.
- Ability to work independently, use initiative and contribute effectively within a team.
- Commitment to continuous improvement, learning and delivering high‑quality patient care.
- Experience within a hospice, community, acute or wider healthcare setting.
- Experience of delivering training, education or facilitation sessions.
- Knowledge of quality improvement methodologies and national guidance, such as NICE and NHS Patient Safety frameworks.
- Experience of service development, change management, research or service evaluation.
- Experience of supporting patient and public involvement activities.
