Job summary
We are looking for an experienced and compassionate Registered Nurse or Allied Health Professional to join our growing SinglePoint of Access team. You will assess and prioritise external referrals into Ashgate Hospice, using your clinical knowledge and professional judgement to identify urgency, need and the most appropriate service response. You will provide advice and guidance to patients, families and healthcare professionals, work closely with our clinical teams and contribute to the ongoing development and quality improvement of the SinglePoint service.
Main duties of the job
You will assess, prioritise and process referrals, using your clinical knowledge and professional judgement to identify urgency, need and the most appropriate service or pathway. You will provide advice, guidance and signposting to patients, families, carers and healthcare professionals, ensuring referral outcomes are communicated and accurately recorded.
You will work closely with Care Coordinators, clinical teams and external professionals across primary care, community and hospital settings to support effective referral pathways and coordinated care.
You will contribute to the ongoing development and quality improvement of the SinglePoint service, including clinical audit, service evaluation and patient feedback, while maintaining high standards of clinical practice, documentation and professional communication.

Ashgate Hospice offers free, specialist, compassionate care for adults living with life-limiting or incurable illnesses, including cancer, neurological diseases, and advanced heart, kidney and lung conditions. Based in North Derbyshire, we support people and those important to them through a range of free and fee-paying services.
Each year, we support over 2,640 people, helping them to manage symptoms, stay informed about their care, and maintain the best possible quality of life from diagnosis onwards. We also provide vital support to those important to them, including practical and emotional support, as well as bereavement care when someone has died.
Job responsibilities
Clinical assessment and triage; referral management; palliative and end of life care; advice and signposting; multidisciplinary and multi‑agency working; care coordination; safeguarding; clinical governance; quality improvement; audit; service development; professional development and reflective practice.
The role is based at Ashgate Hospice, with some flexibility for hybrid working, and is 37.5 hours per week, Monday to Friday, 9am5pm.

Qualifications
Essential
- Registered Nurse (NMC) or Allied Health Professional with relevant professional registration.
- Educated to degree level or equivalent experience.
- Evidence of relevant continuing professional development in palliative or end of life care.
Desirable
- Postgraduate qualification or relevant training in palliative care, leadership or service development.
- Qualification or training in teaching, mentoring, safeguarding, crisis intervention or complex case management.
