Job Description

Sage GroupHealthCare Staffing is seeking Home Health Registered Nurse for a 13-week travel/ contract position in Newport, VT. Accepting both local and travel candidates.

Pay Rate:Range $2280 to $2320 Weekly Gross pay (Non-taxabletravel/housing and M&I stipend available for qualified/eligible travelers)

MileageReimbursement: $0.50/mile for distancecovered between patient home appointments

Unit: Home Health

Shift: 8 Hours; Days, 8a – 5p

Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri, Some weekend oron-call coverage may be required

Position Requirements

  • Registered Nurse: 2 years - Required
  • Recent/Current Home Health Nursingexperience: 2 years - Required
  • OASIS experience: 1 year - Required
  • Must have valid Driver’s Licenseand Personal Auto Insurance
  • Must be Covid Vaccinated orprovide medical or religious exemption

Education, Licenseand Certification Requirements

  • ASN or an equivalent – Required
  • BSN or an equivalent – Highlypreferred
  • RN License for State of Vermontor Multistate - Required
  • BLS Certification from AHA orAmerican Red Cross - Required

Job Description

  • Provide and coordinate care forassigned patients in comfort of their homes.
  • Develop and implement nursingcare plans including Start of Care and admissions as assigned.
  • Support patient recovery andability to stay at home - enabling them to spend quality time with theirloved ones and doing the activities they enjoy.
  • Empower patients and families toreach healthcare goals by educating them on disease management.
  • Case management and provideclinical oversight to interdisciplinary.
  • Provide nursing care and serviceswithin the standards and scope of nursing practice as defined within thestate Nurse Practice Act regulations, policies, procedures, and protocols,as well as facility and corporate policies and procedures.
  • Ensure physicians’ orders arelegible if taken verbally and transcribed appropriately per policy andprocedure.
  • Ensure patients’ status isassessed on an ongoing basis and pertinent information is gathered anddocumented.
  • Notify physicians of significantchanges in status.
  • Document and complete chartingfor all patient visits accurately and in a timely manner.
