- Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments
Position Requirements
- Registered Nurse: 2years - Required
- Recent/Current HomeHealth Nursing experience: 1-2 years - Required
- Must have experiencewith Patient home visits - Required
- OASIS experience: Required
- Ability to complete 5patient visits per day - Required
- Must have validDriver’s License and Personal Auto Insurance
- Must be CovidVaccinated or provide medical or religious exemption
- EMR – McKesson orSimilar – Highly preferred
Education, License and Certification Requirements
- ASN or an equivalent– Required
- BSN or an equivalent– Highly preferred
- RN License for Stateof New Hampshire or Multistate – Required
- BLS Certificationfrom AHA - Required
Job Description
- Ensurespatients have timely and accurate orders prior to delivering services.
- ReviewsPhysician's referral (prescription), patient's condition and medical record.
- Completesan ongoing comprehensive and interdisciplinary assessment including OASIS perregulations.
- Developmentand ongoing evaluation of individual plan of care based on needs, interests,and abilities in conjunction with physician, interdisciplinary team, patient,caregiver and/or representative.
- Effectivelyintegrate educational, medical, social and psychological factors whenevaluating progress towards attainment of goals and outcomes.
- Orients,instructs and may direct work activities of licensed practical nurses (LPN),licensed nursing assistants (LNA), and students as appropriate.
- Performservices/treatments for patient as indicated in plan of care based on physicianorders.
- Complieswith medical, regulatory, educational and records management requirements (e.g.Insurance authorization, consents, physician's referral, documentation) andmaintains patient confidentiality in accordance with HIPAA guidelines.
- Collaboratewith treatment team involved in the patient's plan of care to exchange,discuss, and evaluate patient information for planning, modifying, andcoordinating an appropriate treatment plan and goals.
- Documentsassessment, interventions, medication reconciliation, and discharge planning inappropriate clinical notes and plan of care.Completes documentation same day as visit, per agency standard.
- Participatesin the agency's quality assessment process improvement (QAPI) program toimprove health outcomes, patient safety and quality of care.
