About Us
We are Zetland Medical Practice, a well-established forward-thinking GMS practice comprising 5 GP partners, 3 Emergency Care Practitioners, 2 Practice Nurses, 1 Registered Nurse Associate and 1 HCA supported by an excellent reception and admin team. We are a training practice and currently train GP registrars and medical students. We are a SystmOne practice and a member of Redcar Coastal PCN.
Job Responsibilities
- Support the Practice to deliver safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste, prescribing projects and audits.
- Liaise with local pharmacies and hospitals regarding prescribing queries and optimum therapy to improve prescribing safety, quality and cost effectiveness.
- Support the prescribing clerks in overseeing the preparation of repeat prescriptions, synchronisation of repeat medication, medication review and electronic repeat dispensing.
- Process and update patient medication following hospital discharge and clinic letters.
- Facilitate prescribing meetings relating to practice prescribing reports, audits, protocols, targets, budgets and education sessions.
- Act on and process medication alerts, raising awareness within the practice.
- Oversee patients on shared-care drugs, ensuring recalls and protocols are in place to ensure safety and appropriate use of medicines.
- Support the Practice-Based Pharmacist in providing medication reviews, patient information leaflets and cost-effectiveness of prescribing.
- Maintain accurate patient records, ensuring that all relevant information is documented in the patient’s medical records in a timely manner.
- Keep professionally up to date to meet the GPhC standards for continuing professional development and registration.
- Initiate contact with and respond to requests from patients, other team members and associated healthcare professionals.
- Provide supervision, training and professional leadership to the practice administrative staff.
- Maintain constructive relationships with a wide range of internal and external stakeholders, communicating with all practice staff, secondary care, community pharmacists and professionals.
- Recognise the importance of effective communication with team members, patients and carers, and adapt methods of communication accordingly.
- Adhere to confidentiality and data protection principles, disclosing patient information only to authorised persons in accordance with practice policies.
- Promote and maintain health, safety and security, including personal security systems, risk identification, infection control procedures, PPE, hand hygiene and keeping areas clean and free from hazards.
- Support the equality, diversity and rights of patients, carers and colleagues, respecting privacy, dignity and beliefs, and participating in relevant training programmes.
- Contribute to maintaining quality within the practice, alerting team members to quality and risk issues, assessing own performance, and suggesting improvements.
Qualifications
- Registered with the GPhC as a Pharmacy Technician
- Medicines Reconciliation Certificate or equivalent
- Experience of prescribing/clinical systems such as SystmOne, EMIS Web
Experience
- Recent previous experience working as a Pharmacy Technician
- Recent previous experience within a comparable role in General Practice
