We are looking for a nurse to join our exciting team working on an individual and one-to-one basis with clients, ensuring high standards of care delivery according to a comprehensive individualised care plan. Our Nurse support package could be supporting clients in their transition from hospital to home or supporting a step-down approach in the community setting from a nursing to personal assistant support package. As a member of the team, you will enable our clients' care to be delivered in their preferred place of care. You will be responsible for ensuring the highest possible standard of care is delivered to clients in accordance with the Policies and Procedures of the company through assessing standards against current care regulations and person-centred best practice. The role will be field-based and is part-time at 22.5 hours per week. We are offering a competitive salary of £24,941.76 plus a car allowance of £250 per month pro rata to 22.5 per week.

Main duties of the job:

  • To have individual management responsibility for continuing health care funded care packages in assigned area(s).
  • To ensure care assessments, risk assessments and person-centred plans are completed with clear measured outcomes and within restricted timescales.
  • Assist the Registered managers & Directors to ensure the organisation complies with the Care Quality Commission Regulations, Care Standards Act and Regulations, General Social Care Council codes of practice and other legal requirements.
  • Promote the organisation and network with social workers, health workers and other relevant organisations.
  • To support the onboarding of Personal Assistants, ensuring the right staff team are developed to deliver and support the client and their care needs.
  • Carry out supervisions, team meetings and observations of Personal Assistants and team leaders.
  • To be responsible for delivering training and signing of competencies of those staff completing delegated health tasks, ensuring clients are safe and their needs can be met in their entirety.
  • Direct line management of Personal Assistants.
  • Formulate initial rotas before passing to the Scheduling Department.
  • To participate in the formal and informal training as appropriate to maintain an awareness of current developments and best practice in the field.
  • Use all internal bespoke and generalised IT systems to capture information accordingly.
  • As part of a team rota, offer a 24-hour on-call assistance to clients and staff outside of office hours.

  • Have experience and transferable skills to deliver high quality care and training to our clients and Personal assistants in:
  • Oxygen therapy.
  • Cough Assist.
  • Invasive Ventilation.
  • Noninvasive ventilation.
  • Medication / Pain management.
  • Epilepsy management / rescue medication support.
