REPORTING TO: Service Manager

RESPONSIBLE FOR: Housing Support Worker, Move-On Support Worker, Tenancy Sustainment Support Worker, Referrals and Migrant Move-On Coordinator Worker and other move-on / resettlement roles as required.

PURPOSE OF POSITION

The Move-On Team Leader will provide day-to-day leadership of the Move-On Team, ensuring residents across Your Place services are supported to progress towards safe, suitable and sustainable accommodation.

The role will lead the development of a clear move-on pathway that starts early in the resident journey, supports residents to prepare for independent living, and strengthens tenancy sustainment after move-on.

The post-holder will work across services with residents, key-workers, Service Managers, Team Leaders, Resident Services, Housing Management, commissioners, landlords and external agencies to remove barriers to move-on and improve outcomes.

The role will be responsible for creating and maintaining landlord partnerships, developing move-on routes, coordinating personalised move-on offers for residents, and monitoring move-on and tenancy sustainment outcomes.

This role provides specialist leadership for the full move-on journey, from early preparation through to sustaining accommodation after residents leave Your Place.

It is not just about identifying vacancies or completing referrals; it is about creating a purposeful pathway that helps residents become ready, supported and confident to move into independent or more suitable accommodation.

  • Dedicated move-on leadership: giving clear ownership to move-on, tenancy sustainment and resettlement pathways, with stronger accountability and performance monitoring.
  • Working across all services: embedding move-on planning early and consistently within support planning, rather than waiting until residents are ready to leave.
  • Developing housing pathways: strengthening routes into private rented accommodation, Clearing House, social housing nominations, specialist accommodation, resettlement flats, shared accommodation and other suitable options.
  • Building landlord partnerships: developing relationships with reputable landlords and housing providers, including work around affordability, property suitability, standards and resident readiness.
  • Personalised move-on offers: ensuring each resident has a realistic move-on plan based on their needs, eligibility, affordability, risks, strengths and tenancy readiness.
  • Tenancy sustainment focus: overseeing post-move support so residents can settle, manage finances, set up utilities, connect with local services and address early tenancy risks.
  • Reducing repeat homelessness: strengthening early intervention and tenancy sustainment to reduce tenancy breakdown, eviction and repeat homelessness.

The role will require occasional evening and weekend work in line with service needs.

RESPONSIBILITIES & ACCOUNTABILITIES

Key Responsibilities

  • Lead day-to-day coordination of the Move-On Team, ensuring residents across Your Place receive timely, structured and personalised move-on, resettlement and tenancy sustainment support.
  • Develop and maintain a clear move-on pathway from assessment, induction and early support planning through to move-on and post-move tenancy sustainment.
  • Embed move-on planning into keywork, support plans, tenancy readiness work and case reviews, providing specialist advice to managers, Team Leaders, keyworkers and Resident Services colleagues.
  • Ensure each resident has a personalised move-on plan based on need, eligibility, affordability, risk, strengths, aspirations and tenancy readiness, with clear actions, timescales and owners.
  • Support residents and staff to understand realistic housing options, affordability, benefit implications, shared accommodation expectations, tenancy responsibilities and required move-on documentation.
  • Develop landlord and housing pathway partnerships, including private rented, shared accommodation, social housing nominations, Clearing House, specialist accommodation, resettlement flats and other suitable routes.
  • Maintain pathway information, eligibility criteria, referral routes, rent levels, accommodation types, support expectations and key contacts, and negotiate practical move-on arrangements where appropriate.
  • Oversee post-move tenancy sustainment support, including settling-in support, utilities, council tax, benefits, budgeting, furniture, local services, community connection and early tenancy risk intervention.
  • Monitor tenancy sustainment outcomes, planned and delayed move-Ons, long-stayers, pathway barriers, unplanned exits and repeat homelessness risks, using learning to improve preparation and support.
  • Provide day-to-day leadership, supervision, coaching to the Move-On Team, allocating work, monitoring case records, trackers, action plans and outcome evidence.
  • Maintain oversight of move-on data and support reporting to the Service Manager, Head of Services, SMT and commissioners on outcomes, blockages, performance and tenancy sustainment.
  • Work across Support services, Resident Services, Housing Management, Income, Community Partnerships and Quality Assurance to coordinate a one team around the resident approach.
  • Ensure move-on planning considers safeguarding, mental health, substance use, exploitation, domestic abuse, self-neglect, financial vulnerability, tenancy failure risks, consent, confidentiality and data protection.
  • Apply trauma-informed and proportionate practice where residents decline suitable options or struggle to engage, ensuring decisions, escalation actions and risk information are clearly documented.
  • Contribute to Your Place move-on strategy, keep up to date with housing options, welfare benefits, LHA rates, affordability and legislation, and represent Your Place professionally with partners.
  • Participate in supervision, appraisal, training, reflective practice, organisational meetings, flexible cross-service/community work and any other reasonable duties requested by the Service Manager or Head of Services.

PERSON SPECIFICATION

The successful candidate will bring the experience, skills, knowledge and values needed to lead a specialist move-on, resettlement and tenancy sustainment service across Your Place.

Experience

  • Experience of working with people affected by homelessness, rough sleeping, housing insecurity, trauma or multiple disadvantage.
  • Experience of move-on, resettlement, tenancy sustainment, housing advice, supported housing or private rented sector access work.
  • Experience of supervising staff, coordinating workloads or providing day-to-day operational leadership.
  • Experience of working with landlords, letting agents, housing providers or accommodation pathways.
  • Experience of supporting residents to understand tenancy responsibilities, affordability, budgeting, benefits, utilities and independent living skills.
  • Experience of partnership working with local authorities, homelessness services, housing providers, health services, DWP, voluntary sector agencies or specialist support services.
  • Experience of casework recording, action planning, risk assessment, outcome monitoring and using data to improve service delivery.

Skills and Knowledge

  • Strong understanding of homelessness supported accommodation, move-on pathways, tenancy sustainment and private rented sector access.
  • Good knowledge of affordability, Local Housing Allowance, shared accommodation barriers, landlord expectations and tenancy readiness.
  • Ability to build and maintain relationships with landlords, housing providers, local authorities and external partners.
  • Strong communication, negotiation, advocacy, coaching and organisational skills, with the ability to manage competing priorities.
  • Ability to analyse data, identify trends, produce clear updates and keep actions on track.
  • Good understanding of safeguarding, risk management, confidentiality, GDPR and professional boundaries.
  • Confident using case management systems, Microsoft Office and tracking tools.

  • Experience of working with residents with migrant backgrounds, no recourse to public funds, immigration-related housing barriers or complex eligibility issues.
  • Experience of designing resident-facing move-on workshops, tenancy readiness tools or pathway guidance.
  • Relevant housing, homelessness, advice, social care or leadership qualification.
