Job summary
We are seeking an experienced, enthusiastic and motivated Senior Nurse Practitioner to join the Urgent Care & Support Service, delivering both urgent and proactive care to residents living in nursing, residential and learning disability care homes across Kingston.
The service operates as part of the 2-Hour Urgent Community Response (UCR) Service, providing urgent assessment, treatment and support for residents experiencing acute deterioration in their health and wellbeing, with the aim of preventing avoidable hospital admissions and supporting residents to remain safely within their care home wherever clinically appropriate. Alongside urgent response activity, the service delivers proactive interventions focused on anticipatory care planning, frailty management, long-term condition support and improving outcomes for residents with complex needs.
As a Band 7 Senior Nurse Practitioner, you will provide advanced clinical assessment, diagnosis, treatment and independent prescribing, working autonomously and collaboratively across multidisciplinary teams. You will act as a senior clinical resource, supporting complex clinical decision-making, coordinating care, and leading improvements in quality, education and service development across the care home sector.
Main duties of the job
You will provide advanced clinical care to residents with complex physical, mental health, frailty and long-term care needs, supporting timely assessment, diagnosis, treatment and ongoing management within care home settings.
Working as a senior practitioner, you will lead proactive case management, complex clinical reviews and admission avoidance interventions, helping residents receive the right care at the right time while reducing unnecessary hospital attendances.
You will support residents, families and care home staff with Advance Care Planning, Universal Care Plans, treatment escalation decisions and end-of-life care planning, ensuring care remains personalised and aligned with individual wishes.
The role also includes participation in clinical governance, service evaluation, quality improvement and audit activity, using evidence-based practice and outcome data to improve resident care and shape future service development.
This is a permanent post for 37.5 hours per week. Usual working hours vary depending on service need but are typically Monday to Friday, 8:00 am 6:00pm
