The community Rapid Response team is part of the 2-hour Urgent Community Response service. This is an admission avoidance service for people in the community experiencing a sudden deterioration in health and wellbeing. We are looking for an enthusiastic and experienced nurse or allied-health professional, who can work autonomously in the community. They must be able to demonstrate advanced clinical skills, including physical assessment, clinical decision making and differential diagnosis for a range of common acute and chronic conditions. Candidates must also have an interest and experience in service development and leadership.

This is a permanent post and is for 37.5 hours per week, to be worked over 4 days per week. The usual working hours are 9.5 hours shifts between the hours of 0800 and 2000. This role will be required to work weekends. You will be mostly based at Hollyfield House in Surbiton, although you will also need to be able to travel to our other sites as needed and to community visits. Main duties of the job:

  • Visiting people in their own home, who require urgent assessment and intervention with a view to preventing unnecessary hospital admission.
  • Triage of Rapid Response referrals, allocation to the team and signposting to other services.
  • Working collaboratively with multidisciplinary teams to manage complex health and social care needs.
  • Contributing to audit, service development and quality improvement initiatives.
  • Contributing to leadership and supervision for the team, junior colleagues and students.

Your Healthcare (YH) Rapid Response Team is an integral part of the multi-disciplinary Urgent Community Response Service. The team responds within 2 hours to adults in their own home, who are experiencing an acute or sudden deterioration in their health and wellbeing, with the aim of preventing an unnecessary hospital admission. The team provides advanced clinical assessment, diagnosis, management plans and integrated working with health and social care services to support people at home.

This is an opportunity for an experienced and motivated autonomous practitioner to join an independently led supportive team which is highly valued by health and social care partners across the borough of Kingston. The post holder will demonstrate safe, competent, advanced clinical assessment and decision-making skills and apply a broad range of clinical knowledge and interventions. They will provide leadership support, contribute to service delivery and supervise junior members of the team.

Knowledge:
