We are seeking qualified Nurse Practitioners to service patients in the Gaston area. As a Provider, you play a critical role in delivering exceptional patient care in patients' homes. You are an integral part of the care team, collaborating with multiple healthcare professionals to ensure comprehensive and effective patient management.
Your responsibilities include:
- Evaluating high-risk patients, coordinating care plans, and providing care management to improve patient outcomes.
- Participating in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.
- Documenting care management activities accurately and efficiently to support patient care plans and regulatory compliance.
- Providing patients with education and resources related to their conditions, treatment plans, self-management strategies, and preventive care measures.
- Adhering to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
This is a full-time salary-based position, with the opportunity to receive a monthly bonus.
