Community Health Worker – Facilities, CNA, LPN, EMT

We are looking for a dedicated candidate to join our team as a Community Health Worker (CHW) to service our patients in the Piedmont area. A CHW has a good understanding of the community served, acting as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

Community Health Workers perform visits at ALFs and ILFs in their designated service area. This is a full-time, salary-based 12-hour position (6:00AM-6:00PM) (2,2,3 Schedule).

What Are We Looking For?

Your Health is seeking a Community Health Worker to join our growing urgent care team. The Community Health Worker acts as a liaison between patients, health and social services, and the community to facilitate access to services and improve service quality. The role involves serving approximately 200 patients, collaborating with the care team and health coaches, providing patient/customer services, documenting findings accurately, and communicating effectively.
