We are looking for a dedicated candidate to join our team as a Community Health Worker (CHW) to service our patients in the Chester area. A CHW has a particularly good understanding of the community served, acting as a liaison between health and social services and the community to facilitate access to services and to improve the quality and cultural competence of service delivery.
Community Health Workers perform visits in homes and facilities (ALF and ILF\’s) in their designated service area. This is a full-time, salary-based position working 12-hour shifts (7:00AM-7:00PM).

Your Health is currently seeking a Community Health Worker to join our growing urgent care team. The Community Health Worker serves as a liaison between patients and health and social services and the community to facilitate access to services and improve the quality of service delivery. The role involves serving approximately 200 patients, collaborating with the care team and health coaches, providing patient/customer services, documenting findings accurately, and communicating effectively.
