We are looking for a dedicated candidate to join our team as a Community Health Worker (CHW) to service our patients in the North Augusta area. A CHW has a particularly good understanding of the community served, acting as a liaison between health and social services and the community to facilitate access to services and to improve the quality and cultural competence of service delivery.
Community Health Workers perform visits in homes and facilities (ALF and ILF\’s) in their designated service area. This is a full-time, salary-based position working 12-hour shifts (7:00AM-7:00PM).

What Are We Looking For?
We are seeking a Community Health Worker to join our growing urgent care team. The CHW serves as a liaison between patients, health and social services, and the community to facilitate access to services and improve service quality. The CHW will serve approximately 200 patients and collaborate with Senior Health Express (SHE) to provide various services. Responsibilities include coordinating with the care team and health coaches, providing patient/customer services, documenting findings accurately, and communicating effectively.
