Live Well Officer - Grade 6 (£34,416 - £35,448) - 12 Months Fixed Term/Secondment Croydon Council’s Live Well service is an integrated health behaviour change 12-week programme that offers residents help and support to maintain a healthy weight, stop smoking, and drink less alcohol. Smoking, obesity, physical inactivity, and excessive alcohol consumption are responsible for around 30% of the disease burden and are associated with major causes of morbidity and mortality. These behaviours are influenced by complex interactions between a person’s social, economic, and environmental circumstances. We are seeking a motivated candidate to be the main point of contact within our triage team for the Live Well Service. The postholder will be responsible for contacting and triaging all referrals, entering details into our bespoke database, along with various admin tasks. The position is based within the Adult, Social Care and Health Directorate. The role aims to support the Live Well coordinator in organising and supervising the delivery of the service, ensuring its integration across the Croydon locality. The postholder will provide project management support and be responsible for the day-to-day planning and implementation of the service. Additionally, they will assist the Live Well Croydon Coordinator with programme reports and data outcomes related to service delivery. This role requires extensive teamwork, initiative, judgment, confidentiality, and diplomacy.
