At Younger People with Dementia (YPWD), we support adults diagnosed with young onset dementia (diagnosed under the age of 65) to remain active, connected and independent through meaningful activities, one-to-one support and community programmes.

This isn't a personal care role. It's about helping people continue to live well, enjoy life and do the things that matter most to them.

As part of our friendly and supportive team, you'll help deliver a wide range of engaging activities, from art, walking and singing to katakanuing, reader groups and community outings.

You'll also provide one-to-one support, help plan future sessions and complete the records that enable us to deliver safe, high-quality services.

No two days are the same, and we're looking for someone who enjoys variety, uses their initiative and loves helping others have fun.

Please note that you must have a full UK driving licence with daily access to a roadworthy vehicle insured for business use.

Key Responsibilities/Activities

  • Support individuals where possible to boost self-esteem and confidence, and to learn new skills whilst promoting choice and independence through our services.
  • Plan, prepare, risk assess and evaluate such activities/workshops and one-to-one support for both face-to-face work and on live video platform.
  • Provide full proactive, active and reactive support to service users through use of suitable evaluation tools and steering groups.

Continually review services and adjust accordingly to the needs of people accessing the service.

  • Establish good working relationships with existing organisations that already offer respite to people with young onset dementia in the local area.
  • Ensure the service offered is accessible, user friendly, age-appropriate, activity based, stimulating and where appropriate, research based.
  • Provide one-to-one support to those with young onset dementia where appropriate. This support could take place in their home, community venues or via video call.
  • Be flexible in the planning and running of workshops, adapting as may be required.
  • Ensure that both written and verbal communication conforms to the highest professional standards at all times and maintains the dignity and respect of service users.
  • Ensure ongoing assessment and management of risks associated with attendees are within an attitude of 'positive risk taking'.
  • Attend regular supervision with line manager and memory clinics where appropriate.
  • Demonstrate responsibility and leadership for promoting and championing all aspects of equal opportunities by valuing diversity in all areas of work.
  • Undertake general administrative duties where required, including the completion/updating of attendees' records.
  • Undertake any other duties in order to meet personal, team and organisational objectives following consultation with your manager.
  • Complete both mandatory and internal training as and when directed.

Qualifications and Experience

  • Hold, or be willing to work towards, a Level 2 qualification in Health and Social Care (or an equivalent qualification).
  • Experience of supporting adults in a health, social care, community or wellbeing setting.
  • Experience of planning and facilitating engaging activities, workshops or group sessions.
  • Excellent interpersonal and communication skills, with the ability to build positive relationships with people living with young onset dementia, their families, volunteers and partner organisations.
  • Strong organisational skills, with the ability to prioritise tasks, manage your workload and maintain accurate records.
  • Confident using IT, including Microsoft Outlook, Word and Microsoft Teams and willing to learn new systems.
  • A commitment to person-centred practice, promoting independence, choice, dignity and positive risk-taking.
  • A positive, collaborative and solution-focused approach, with the ability to work effectively both independently and as part of a team.
  • Full UK driving licence with daily access to a roadworthy vehicle insured for business use.
  • Willingness to travel across Berkshire, Surrey Heath, Farnham and North-East Hampshire.
  • Experience of supporting adults in a health, social care, community or wellbeing setting.
  • Experience of planning and facilitating engaging group activities, workshops or community programmes.
  • Experience of supporting people living with dementia, or other long-term neurological or cognitive conditions.
  • An understanding of the impact of young onset dementia on individuals and their families.
  • Experience of working collaboratively with community organisations, volunteers or partner agencies.
  • An understanding of safeguarding, confidentiality, professional boundaries and person-centred practice.
  • Experience of recording outcomes, maintaining accurate records or demonstrating the impact of support provided.
