The Young Wellbeing Hub is a multidisciplinary clinic dedicated to supporting the mental health and wellbeing of young people. We provide accessible, high-quality care and work collaboratively across services to ensure that every young person is guided to the right support at the right time. Our team is made up of compassionate clinicians, administrators, and support staff who are passionate about improving outcomes for young people.

We are looking for a motivated and empathetic Patient Pathway & Clinical Support Coordinator to join our growing team. This is a hybrid role combining administration, patient-facing responsibilities, and direct clinical support.

You will be a key point of contact for young people accessing our services, supporting them through their journey from initial enquiry to assessment. You will also work closely with clinicians, helping to prepare for assessments by gathering screening information and ensuring appointments run smoothly.

Ideal for:

Key Responsibilities:

  • Act as a first point of contact for patients and families via phone, email, and in person
  • Support patients who are currently accessing the service, answering queries and providing guidance
  • Help patients understand available services and support them in choosing the most appropriate pathway
  • Book, manage, and coordinate appointments efficiently across clinicians and services
  • Clinical & Assessment Support:
  • Support clinicians by collecting screening information and assessment questionnaires
  • Liaise with patients to ensure all required information is gathered prior to assessments
  • Prepare clinicians for upcoming assessments by ensuring records and screening data are complete and organised
  • Maintain accurate and confidential patient records in line with data protection requirements
  • Administrative Duties:
  • Provide high-quality administrative support to the clinical team
  • Manage referrals, correspondence, and documentation
  • Support the smooth day-to-day running of the clinic
  • Work collaboratively with colleagues to continuously improve patient experience and clinic processes

Person Specification:

  • Excellent communication skills, with the ability to engage sensitively and professionally with young people and families
  • Would suit an ambitious graduate that can learn fast or someone that already has experience in the sector
  • Strong organisational skills and attention to detail
  • Confidence managing multiple tasks and priorities in a busy clinical environment
  • A professional, compassionate, and patient-centred approach
  • Ability to work full-time from our Hackney Wick offices

Desirable:

  • Experience in healthcare, mental health, or clinical administration
  • Background as a medical secretary, healthcare administrator, or care coordinator
  • A relevant degree (e.g. psychology, health sciences, social sciences)
