The Young Futures Partnership (YFP) is made up of five supported accommodation providers across Cambridge and operates through a single referral pathway. This role will take the lead on all referrals received through that pathway, ensuring a consistent, trauma-informed approach to young people's assessments and working collaboratively with all relevant parties on assessment outcomes. The role will also support the YFP's commitment to putting service users at the centre of the service, empowering young people to achieve their goals and aspirations.

Responsibilities

  • Lead on all assessments (with support from each YFP service) to ensure a consistent approach across the partnership.
  • Monitor incoming referrals and respond to all enquiries within an appropriate timescale.
  • Liaise with YFP services to agree placements once assessments have been completed.
  • Support young people using a strength-based approach, carrying out person-centred assessments that promote choice and control.
  • Complete assessment write-ups promptly to maintain accuracy and quality of information.
  • Manage administrative tasks within In-Form (the data management system), including assisting with the collation and submission of information for performance monitoring and contractual reporting.
  • Communicate effectively with referring organisations to ensure they understand the remit of the YFP, including the types of accommodation and support provided by each service.

Days and Hours: 16 hours a week, Monday 9am - 2pm, Wednesday 9am - 3pm and Friday 9am - 3pm.

Salary/Hourly rate: £10,458.24 per annum, additional benefits we offer include:
