Responsibilities
We are looking to recruit a Health Link Worker who will be based at our Cornerstone Day Centre, a community centre for people who are homeless or at risk of homelessness. The Health Link Worker will facilitate the Homeless Health Hub, speaking to people who come into Cornerstone about our health offer and encouraging them to take it up. Your role will be to organise and run health promotion and wellbeing activities which may include gardening, health walks or creative activities. You will support and motivate people who use the service to see the onsite health professionals or access mainstream health services.
Qualifications
You will have excellent customer service and IT skills, with the ability to motivate and support people to make positive health choices and lifestyle changes. Experience of promoting health care and wellbeing and working with people who are homeless or people with substance or mental health problems in a community setting is essential.
