Responsibilities:

  • Using professional case management practice, carries out intake assessment, service coordination, referrals, case conferencing and case monitoring.
  • Provides supportive counselling to clients and their family caregivers on an individual and group basis.
  • Coordinates with interdisciplinary team members to carry out and monitor resident care plan pertaining to social work support.
  • Offers instrumental help to assist the client to resolve functional problems of daily living.
  • Provides crisis intervention and short term assistance in addition to maintaining a caseload assignment.
  • In consultation with the Executive Director, develops and implements policies pertaining to social work services.
  • Participates in and contributes to interdisciplinary team conference of the LTC Home.
  • Works closely with the Executive Director in providing social work services to residents of the LTC Home.
  • Actively participates and contributes to the overall LTC leadership team meetings and functions.
  • Keeps up-to-date with developments and best practices in the field.
  • Other duties as assigned.

Qualifications:

  • Bachelor degree in Social Work, or equivalent, and Registered Social Worker designation in Ontario.
  • Minimum 2 years experience in counseling or group work, preferably in a long term care environment.
  • Proficiency in computer applications.
  • Ability to work independently, be self-directed to problem solve, exercising good judgment, tact and diplomacy.
  • Empathetic attitude and desire to contribute towards the wellbeing of the elderly.
  • Excellent communication and interpersonal skills.
  • Satisfactory Police Vulnerable Sector Screen result.
  • Ability to converse in Chinese.

Salary Range: $34.28 - $40.32
