About Aios Medical
Our overarching mission is to build the AI operating system for consumer healthcare - delivering end-to-end care with near-zero marginal human time. We are not an average healthcare company. We believe the future of healthcare is automated, intelligent, radically lower cost, and built around the consumer. We believe biology is entering a golden age. We believe profitable, focused companies beat bloated ones. We're determined to get the humans out of healthcare - replacing slow, expensive, legacy workflows with software-driven care delivered at scale.
We’ve started by building this infrastructure to power our own brands - including Bolt Pharmacy and Fella Health, among others. Today, that infrastructure is helping patients access life-changing treatment for excess bodyweight. Over time, we’ll extend it across many of the categories that will define the next era of healthcare. Our infrastructure will have served 100 million patients worldwide by 2035. We’ll then use that infrastructure to deliver next-generation healthcare products at massive scale.
How far along are we? We’ve grown 40x over the past 12 months, now serving 100,000+ patients and surpassing $200M in annual revenue, all while staying cashflow positive. We’re a young startup and we’re just getting started. This is still Day 1 and all our work is ahead of us.
We're backed by top healthcare & consumer investors:
- Y Combinator
- Global Founders Capital
- AngelList Early Stage Fund

Being a Fractional UK Medical Lead at AIOS Medical
We’re building a world-class team. As Fractional UK Medical Lead at Aios (powering Bolt Pharmacy), your fundamental role is to build and scale a prescribing model for our medic team that is safe, compliant, and commercially strong. You’re an obesity management/endocrinology specialist and use your deep expertise to build and own the clinical logic behind the system.
While you won’t manage the medic team directly, you’ll help shape the standards they operate within: contributing to governance and audits, reviewing edge cases, and strengthening safety frameworks so quality remains painfully high as volume increases. Stellar performance in this role means the medic team scales compliantly without breaking. Patients move through care smoothly and the system you build supports growth.
If you nail this role, you’ll define how modern, protocol-driven care is delivered to millions. Then, you’ll explore expanding that model across new treatments. This is a senior role, and is part time and fully remote (based in the UK). You’ll be reporting directly to our Interim Clinical Lead, Saim Dalvi.
You’ll also work closely with:
- Ben Dowdle (Head of Product)
- Phuong Ngyuen (Superintendent Pharmacist)
- Jordan Pellikan (Head of CX & Ops)
Key Responsibilities
- Safe, high-quality patient consultations
- Deliver and oversee asynchronous and synchronous consultations with high documentation standards, applying sound knowledge of online prescribing risks including safeguarding, triage, and fraud prevention to maintain strong compliance whilst enabling strong growth.
- Ensuring complex patients get the right care
- Support patients who need more complex GLP-1 management, ensuring they receive thorough, personalised care
- Build the frameworks and escalation routes to help clinicians make confident decisions in complex cases
- Support audits and edge case reviews, ensuring standards hold as volume grows
- Own off-licence GLP-1 protocols
- Build eligibility, titration, and safety frameworks
- Ensure protocols are compliant, defensible, and effective
- Ongoing clinical logic into Product
- Stay current with emerging evidence and update protocols accordingly
- Turn protocols into clear, executable rules
- Continuously improve protocols using real data
Need to Have
- UK medical licence: You understand prescribing within a UK private/CQC context and maintain an active licence to practice.
- Clinical background and expertise: You bring experience in weight management or metabolic health. You have a solid familiarity with GLP-1 receptor agonists and their system wide effects.
- Telehealth experience: You understand how remote prescribing actually works in practice (both asynchronous & synchronous).
- Frontier awareness: You actively track emerging evidence, new treatments, and evolving best practices. You have strong thoughts on where care is going in the AI age.
- Clinical judgement at scale: You’re comfortable owning, making, and refining clinical decisions that apply across large patient populations.
- Governance exposure: You’ve been involved in audits, protocol reviews, and clinical decision frameworks. You understand what “defensible” looks like under scrutiny.
- Speed + pragmatism: You move quickly to make clear, confident decisions rooted in deep expertise. You expertly balance safety with execution to help improve patient outcomes rapidly.
- Patient-centric: You understand how protocol-design decisions impact scalability and the overall patient experience.
Nice to Have
- Off-licence GLP-1 protocol design: You’ve directly contributed to designing off-licence prescribing protocols (GLP-1 / weight management).
- CQC / regulated telehealth leadership: You’ve led within UK private care frameworks and understand how to keep a large operation compliant.
- Product-minded clinician: You’ve worked closely with product/engineering teams before to turn logic into software, not just documents.
- Startup pace: You’ve operated in high-velocity environments where priorities shift often.
- Systems thinker: You instinctively simplify complexity, remove ambiguity, and design frameworks that scale beyond individual judgement.
- Builder mode: You may have built something previously, either founding your own private practice or being the first Medical Officer at a startup.
Our cultural standards
These are the core cultural standards to which we hold ourselves & our team-members at Fella:
- Belief in the mission: We will have served 100 million patients by the end of 2035 and we transform the life of most patients who join. We have a lot of work to do. We are obsessed with our patients and are dedicated to the mission.
- Unwavering integrity: We are at the frontier, so we often live in ambiguity with no trodden path. When we can’t look to others for guidance, we must maintain impeccable ethics and unwavering integrity.
- Only the paranoid survive: Bad sh*t is coming. By joining us, you’re choosing to sail straight towards the storms with unhesitating conviction. However much we’ve already done, however far we’ve already come — it’s still Day 1 and all our work is ahead of us.
- If we’re average we fail: We are only interested in “insanely great”, a focus on the quality of our execution that in everyday life would be considered pathological. We have a dedication to excellence and reject incompetence.
- Commitment to candor: That which can be destroyed by the truth should be. You get full transparency from the company and the company expects full transparency from you. We never say anything about someone that we wouldn’t say to them directly. We give feedback with love and do not need to protect people from fleeting physical sensations.
- A maniacal sense of urgency: We execute at an intensity that most people think is impossible. Speed is critical and we need things done yesterday. We all work very hard and in such a competitive world there really is no other way to win.
- Enduring frugality: We are frugal. We hate being wasteful and we are anti-luxury. A culture of cheapness keeps us young. We spend our cash wisely & carefully — in a way that would make our grandmas proud.
- Bulldozing barriers: The world is malleable and we shape it. We truly believe this and act accordingly. We are relentlessly resourceful and are at the mercy of no-one but ourselves. You’ll be shocked how capable you are and how much you can achieve.
- Keep your head down: We’re boring people doing exciting work. We don’t chase short-term status — we ignore short-term dopamine hits and focus on what matters. Outsiders will underestimate us and we revel in that.
- The power of focus: We live in a world of power laws and we cannot overestimate the unimportance of practically everything. Know your One Thing, and nail it.
