YAI Network in Kingston is seeking a START Coordinator to support individuals with intellectual/developmental disabilities and co-occurring behavioral health conditions, delivering evaluations, resources, and discharge planning in collaboration with the interdisciplinary team.
You will maintain a caseload of up to 25 clients, conduct in-person and telehealth assessments, coordinate crisis plans, attend regular meetings, and provide on-call coverage while ensuring START model fidelity.
