We have an exciting opportunity for two enthusiastic and motivated dietitians to join the dietetic team at Sheffield Teaching Hospitals NHS Foundation Trust. This role will work within the Sheffield Community and provide a service to the Sheffield Community. This post will also involve supporting our Community Dietetics Improvement Programme. The STH dietetic department consists of around 65 Dietitians, plus dietetic assistants and admin staff across 3 hospital sites and Sheffield community. It is a friendly, supportive, and innovative department with a commitment to patient‑centred care and best practice. The successful candidate will be able to demonstrate strong clinical skills, excellent organisational and interpersonal skills.

Main duties of the job:

  • Determine the appropriate dietetic treatment and develop effective management plans for patients within the community setting with a variety of conditions and dietetic diagnoses (e.g., malnutrition, gastrointestinal conditions, diabetes).
  • Conduct dietetic assessment and calculate dietetic requirements of referred patients considering medical, biochemical, social, cultural and psychological factors.
  • Provide nutrition and dietary advice suitable to the patient’s medical, biochemical, social and cultural factors based on current evidence, incorporating local professional and national guidelines and standards.
  • Negotiate with patients to enable them to achieve targets, overcoming barriers to change including dietary, environmental and social factors.
  • Monitor and review dietetic care plans against outcomes.
  • Manage and adjust care plans to facilitate achievement of dietetic goals.
  • Ensure dietetic interventions can be implemented through continued collaborative interdisciplinary working in and outside the Trust by attending and participating in relevant community multidisciplinary meetings, best interest meetings and case conferences.
  • Provide dietetic support in a variety of settings: in patients’ own homes, face‑to‑face in clinics, telephone clinics and virtual clinics.

Person specification:

Education and Qualifications:

  • BSc Nutrition and Dietetics or Post‑Graduate Diploma in Dietetics
  • Current HCPC registration
  • BDA accredited clinical supervisory skills course for training students
  • BDA accredited Behavioural change, counselling, motivational and cognitive behavioural skills course

Experience:

  • Substantial dietetic experience in a variety of clinical specialities
  • Evidence of independent working
  • Good knowledge of a range of clinical conditions and current best practice for dietetic management, especially Nutrition Support
  • Competent in behavioural change skills
  • Previous clinical experience of working within the Community setting

Skills and Knowledge:

  • Accurate documentation and record‑keeping in line with national standards
  • Manual and electronic recording of activity and other data
  • Development of educational resources to meet varying client and carer needs, evidence‑based
  • Aware of clinical governance and its implications for dietetics including importance of audit projects
  • Use of literature review search methods and critical appraisal of evidence
  • Knowledge of regulatory bodies and professional bodies codes of practice and professional guidelines
  • Competent skills in teaching and training other health care professionals and student dietitians
  • Management of a caseload with frequently changing priorities and resilience to interruptions
  • Ability to work as an integral member of an MDT and as an autonomous practitioner

  • Excellent interpersonal, communication and team‑working skills
  • Professional approach to relate and communicate clearly at all levels with patients, colleagues and MDT members
  • Evidence of continuing professional and personal development and commitment to continued progress through departmental appraisal system
  • Willingness to undergo further training at post‑registration level
  • Car driver and access to own transport
