Advanced Clinical Support Worker (ACSW) in Sheffield

The post holder will work as part of the multi-disciplinary team delivering person-centered care that respects dignity, is non-judgmental and value-based encompassing the six Cs: (Care, Compassion, Competence, Communication, Courage, and Commitment), with care focused on supporting patients to self-manage their healthcare needs wherever possible.

Working alone or in ‘twos’ where required, deliver clinical and non-clinical care in line with the designated care plan prescribed by a registered practitioner. Follow the care plan in the patient’s home, escalating any concerns to senior clinicians where a patient’s situation changes or condition deteriorates. Undertake clinical observations as required using manual and/or electronic methods and elevate any observations to the clinician that fall outside the stipulated guidance. Take the appropriate action as instructed by the registered clinician. This may include referral to other agencies such as the District Nursing or Safeguarding. Assess and identify any issues or concerns regarding any aspect of the patient or the patient’s care and report these findings accurately both verbally and in writing using the relevant IT systems, to the multi-disciplinary team.

The postholder will assist the multi-disciplinary team of nurses & other AHPs, medical staff & GPs, to provide care within the Community Integrated Care Service and Stroke Pathway, by following designated treatment plans providing and managing ongoing treatments on a daily basis; liaising with other team members and/or healthcare professionals as required. The post holder will implement packages of care for patients to enable them to remain in their home and will work to specific care plans & standard operating procedures (SOPs) with access to support from the clinical team.

Administer medication to patients, including controlled drugs, following the care plan & Medicines Management Administration Report (MAR) chart, or prompt patients to take their own medication. Identify issues such as allergic reactions to medication, or deviation from the care plan and report to the appropriate clinician as directed. Prepare medication to enable patients to self-administer either orally or via PEG where patients have the capacity to self-administer.

The postholder will be expected to work on their own driving to and from patient's homes and able to use an electronic scheduling system via an IT tablet device. We are specifically looking for part-time staff to work predominantly unsocial hours including evenings 4pm-10pm weekdays/weekends/bank holidays and some early shifts 7.30am - 1.30pm on weekends & bank holidays.

Clinical staff work directly at the interface between primary, secondary, intermediate and social care optimising their clinical skills and knowledge to provide seamless transfers of care and enhance the patient experience. Education and Qualifications

• NVQ or BTEC Level 3 in a relevant health-related subject or must be able to demonstrate equivalent level of knowledge, training & experience in providing direct care to patients.
• Able to demonstrate a commitment to lifelong learning supported by evidence in the format of personal development plans and achievements.

Experience

• Able to demonstrate skills and experience of providing high-quality personal care to patients either in a hospital, community, care home, or the patient’s home environment.
• Relevant experience of working with patients who present with complex and multiple pathologies.
• Experience of working in a multi-disciplinary team.

Further Training

• Competence in basic clinical skills relevant to the role.
• Willingness to undertake further training relevant to the role which may be undertaken ‘on the job’ and may include pressure area care, end of life care, and dementia awareness.
• Food hygiene certificate or other training in food hygiene or must be willing to undertake.
• Excellent verbal and non-verbal communication and listening skills – must be able to provide information and advice to patients, relatives, carers, and colleagues.
• Excellent verbal written and interpersonal skills.
• Demonstrable awareness of first aid at work.
• IT literacy including the ability to use electronic patient record systems.

Personal Qualities

• Due to the requirement to undertake visits the post holder is required to have a valid UK driving licence with access to a vehicle or other form of transport, for work purposes.
• Understanding of, and ability to maintain, confidentiality and data security. Awareness of data protection requirements.
• Awareness of cultural diversity.
• Ability to work flexibly and flexible hours when required for the team or service.
