Job Overview
This role is within the Community Nursing team across the Fylde coast. Full‑time, part‑time, permanent and fixed‑term positions are available.
Applicants must be registered nurses with degree‑level qualifications, specialist post‑registration education and leadership training or equivalent.
A minimum of 18 months experience at Band 5 is required.
Main Duties
- Assess, plan, implement and evaluate care for a defined district nursing caseload.
- Ensure care needs are met through effective management and appropriate delegation.
- Work in partnership with the primary health care team, liaising with all involved in holistic patient care.
- Demonstrate extensive knowledge, advanced analytical and practical skills, and empathetic communication with patients, carers and professionals.
- Provide clinical supervision to junior staff and support staff development.
The post holder must possess strong written and verbal communication skills, evidence of ongoing professional development, and be able to work independently.
Detailed Responsibilities
The postholder works within a community nursing service across the Fylde coast, delivering care in patients’ homes or clinic settings. Key responsibilities include:
- Providing safe, effective, individualised care and supporting families and service users requiring scheduled and unscheduled care interventions.
- Monitoring patients’ condition and health needs continually in partnership with colleagues, families and carers, and referring for reassessment when required.
- Maintaining delegated 24‑hour responsibility for a caseload, coordinating admissions and discharges for patients registered to specific GP surgeries, and providing clinical leadership on weekends and bank holidays.
- Assessing patients, planning, implementing, evaluating and reviewing care in home or clinic settings, and maintaining accurate clinical records.
- Administering drugs in line with NMC Policy, including controlled drugs.
- If qualified, prescribing in line with Trust guidelines, prescribing protocols and the Nurse Prescribing Formulary.
- Producing appropriate written reports as requested.
- Assisting in investigation of patient complaints.
- Providing leadership and operational management of a community nursing team, supporting, supervising and managing staff.
- Attending team briefings and staff meetings to disseminate information and implement changes where appropriate.
- Delegating work to members of the Nursing Team, considering Scope of Professional Practice and skill mix.
- Liaising with other health, social care, voluntary and private sector services to ensure a collaborative approach to service provision.
- Contributing to the development of nursing services within the division.
- Organising community schedules for students to provide robust educational placements.
- Providing an induction training package and clinical supervision opportunities for preceptees and new staff.
Person Specification
Education and Qualification
- Registered Nurse Level 1.
- Current NMC Registration.
- Evidence of ongoing Professional Development.
- V100, V150 or V300 Non‑Medical Prescriber or willingness to undertake.
- Supervisor/Assessor Competence.
