We have an exciting opportunity for a permanent Band 6 Specialist Nurse for Children's Continuing Care. The post holder will be responsible for coordinating and managing a caseload of children and young people with complex health needs. The role focuses on delivering high‐quality, child‐centred care through comprehensive assessment, care planning, and multi‐agency working, ensuring care packages are safe, effective, and aligned with national Continuing Care guidance.
Main duties
- Undertake comprehensive Continuing Care assessments and reviews in line with national frameworks
- Coordinate and manage complex caseloads, ensuring timely follow‐up and case oversight
- Develop, implement, and evaluate individualised care packages in partnership with families and the MDT
- Liaise with the ICB, education, social care, and other agencies to ensure integrated care delivery
- Support complex discharge planning and transitions of care
- Provide clinical leadership and guidance within the team
- Contribute to training, supervision, and service development activities
- Maintain accurate documentation and ensure compliance with governance and safeguarding standards
Experience
- Experience managing children with complex health needs
- Experience of assessment and care planning (e.g. Continuing Care / CHC)
- Experience of multi‐agency working
