Main duties of the job:
- Triage referrals from various establishments, wards, CAU/SDEC, tertiary centres and GPs.
- Demonstrate excellent communication skills when dealing with patients, relatives and staff.
- Participate in outpatient clinics, assessing the patients' clinical condition and identifying individual risk factors.
- Provide education and guidance in accordance with the patients’ needs.
- Be responsible for a caseload of patients, including implementing and monitoring the patient’s management plan.
- Maintain clinical competency as required to meet the needs of patient caseload.
- Be competent to analyse the findings of investigations and diagnostic testing and act in the best interest of the patient, for example – echocardiograms, ECGs, blood tests etc.
- Work closely with consultant cardiologists discussing individual cases and taking advice when necessary.
- Adhere to the Nursing and Midwifery Council’s Code of Conduct as well as conforming to Trust Policies, Procedures and Guidelines.
- Refer to different specialities as needed.

Qualification & Education:
- Registered Adult Nurse.
- Evidence of continuing professional development.
Communication Skills:
- Able to present information to both patients and professional groups.
- Ability to develop effective interpersonal relationships with colleagues across health and social care settings.
- Ability to cope with distressing and/or emotional situations.
- Demonstrates respect and dignity towards patients and staff.
- Demonstrates effective and appropriate communication skills (verbal, non-verbal & written).
Knowledge & training:
- Knowledge of invasive cardiac procedures.
- In-depth knowledge of caring for patients with a variety of cardiac conditions.
- Able to demonstrate an expert level of expertise within cardiac care; focusing on cardiac rehabilitation and secondary prevention.
- Able to motivate and educate patients and other health professionals.
- British Association for Cardiovascular Prevention & Rehabilitation (BACPR) Standards and Core Components.
Experience:
- Minimum 2 years cardiac nursing.
- Ability to prioritise and manage own workload.
- Analytical and judgement skills.
- Commitment to evidence based practice, specifically within cardiac care.
- Ability to assess, plan, implement and evaluate care for coronary heart disease patients without supervision.
- Ability to maintain accurate records, data, and information and to produce reports as required.
Management Skills:
- Act as a role model to other health care professionals, especially in a manager's absence.
- Use critical thinking to manage complex issues and drive improvement.
- Excellent interpersonal, motivational, and conflict resolution skills.
