A rare & exciting opportunity has arisen for an enthusiastic qualified nurse with renal experience to join our expanding, dynamic team of renal specialist nurses. The role requires someone able to support the existing & expanding home therapies with skills & experience in Peritoneal dialysis. The role is varied & involves working in collaboration with the multidisciplinary team to provide the highest quality innovative care. You must be flexible in your approach to work; this includes travelling between hospital sites to deliver nurse led clinics and visiting patients at home. Access to a car and possession of a clean driving licence is essential to the post. You will need to be able to work flexible hours to ensure the requirements of the service are covered. This clinical nurse specialist role has delegated responsibility for the case management of Peritoneal Dialysis (PD) Patients within the Trust, including assessment of care needs, development, implementation and evaluation of care and setting standards of quality care. Previous renal experience essential and peritoneal dialysis would be advantageous. The successful candidate will be supported to develop the necessary skills within the role. The Trust has an excellent educational programme supporting personal development in autonomous practice including working towards Advanced Practitioner roles.

Main duties of the job:

  • Educating patients and families, assessing their home for suitability and working with MDT to get any adaptations made.
  • Organising and aiding best practice regarding Catheter placement, care post op.
  • Training patient and support person how to carry out Continuous Ambulatory Peritoneal Dialysis (CAPD) and Automated Peritoneal Dialysis (APD), in hospital and at home.
  • Coordinate & manage the treatment of patients with peritonitis.
  • Organise and run clinics to review patient's treatment, carry out tests, follow up and act upon results to ensure adequacy of dialysis is maintained as well as general well-being and carry out routine procedures e.g. line changes.
  • Act in an advisory capacity to provide support and advice to patients, support person, and nursing staff to ensure that dialysis is carried out safely.
  • Teaching nursing staff to carry out CAPD to assist patients who become unwell and are admitted for acute treatment.
  • Teaching to a group of individuals with differing learning abilities.
  • To develop practice according to standards and best practice guidelines.
  • To work autonomously with a patient group coordinating patient need with other members of the MDT.
  • To value & embrace ongoing education for patients & within the wider MDT.
  • To communicate effectively with all areas of the health care disciplines.

Person specification:

EDUCATION, QUALIFICATION S & TRAINING:

  • Registered Nurse
  • Relevant Speciality course
  • Educated to degree level
  • Teaching / assessor qualification

KNOWLEDGE & EXPERIENCE:

  • Previous experience within the speciality
  • Experience at senior staff nurse level
  • Ability to demonstrate up to date experience and depth of knowledge and clinical competence
  • An awareness of Trust Policies including renal NSF, clinical governance, data protection etc
  • Demonstrate ability and experience in change management/ clinical developments.

SKILLS & ABILITIES:

  • Able to demonstrate team management skills with an understanding of the leadership role.
  • Ability to act on own initiative with evidence of sound decision making abilities.
  • Recent experience within acute Renal setting and competent with dialysis skills.
  • Excellent written, verbal and non-verbal communication skills.
  • Able to demonstrate an active role in teaching.
