Main duties of the job:
- Triage referrals from various establishments, wards, CAU/SDEC, tertiary centres and GP’s.
- Demonstrate excellent communication skills when dealing with patients, relatives and staff.
- Participate in outpatient clinics, assessing the patients' clinical condition and identifying individual risk factors.
- Provide education and guidance in accordance with the patients’ needs.
- Be responsible for a caseload of patients, including implementing and monitoring the patient’s management plan.
- Maintain clinical competency as required to meet the needs of patient caseload.
- Be competent to analyse the findings of investigations and diagnostic testing and act in the best interest of the patient, for example – echocardiograms, ECG’s, blood tests etc.
- Work closely with consultant cardiologists discussing individual cases and taking advice when necessary.
- Adhere to the Nursing and Midwifery Council’s Code of Conduct as well as conforming to Trust Policies, Procedures and Guidelines.
- Refer to different specialities as needed.
