We are looking for an enthusiastic and motivated individual to join our acute medicine pharmacy team at Hull University Teaching Hospitals as an Advanced Clinical Pharmacist. You will provide clinical pharmacy service to a variety of acute medical areas, within a Pharmacy Department that provides a supportive environment for professional development with a range of experienced and specialist senior pharmacists.
Main duties of the job:
- Offer advice to medical, nursing, other healthcare staff and patients on all aspects of the safe & effective use of medicines while providing a clinical pharmacy service.
- Facilitate reconciliation, prescribing advice, discharge planning and patient counselling.
- Support the clinical trials process and prescribe to support medicines safety, effectiveness and discharge flow for pharmacists working at Advanced Level.
- Work with a pharmacy team that includes qualified staff such as technicians and assistants to support day-to-day activities.
- Contribute to a culture focused on patient interests and continual improvement of services, with an emphasis on professional development for the pharmacy workforce.
- Support, motivate and engage with colleagues to promote professional development.
You must be a registered pharmacist with the GPhC, hold a post-graduate diploma or equivalent, and be able to demonstrate evidence of your Continued Professional Development (CPD).
