We are now looking for a highly motivated and experienced Occupational Therapist to join our team. If you have strong expertise in hospital discharge pathways, reablement therapeutic interventions, and are committed to early intervention then this role offers the chance to make a meaningful impact every day.

Responsibilities

  • Support citizens to return home or to the most appropriate environment, facilitating positive risk taking while prioritising safety, wellbeing, and independence.
  • Work as part of a team committed to developing integrated pathways, including Discharge to Assess, to help people regain skills following a hospital or care home stay.
  • Play a central role in strengthening prevention and early intervention at the front door of adult social care, ensuring people receive timely and effective support.
  • Act as a role model within the All-Age Occupational Therapy team, offering mentorship and sharing your specialist knowledge with less experienced colleagues.
  • Work within a supportive structure led by HCPC-registered OT managers who value the profession, champion high-quality practice and provide responsive professional supervision.

An Enhanced DBS check and HCPC registration are required for this role.
