We are looking for an enthusiastic and dedicated Practice Nurse to join our friendly, forward thinking primary care team. This is an excellent opportunity for a motivated nurse who is passionate about delivering high quality patient care and contributing to the ongoing development of our clinical services. The successful candidate will play a key role in providing a wide range of nursing services within the practice, including routine clinics and chronic disease management.
Main duties of the job
- Routine Clinics
- Cervical Screening - Perform cervical cytology in line with national guidelines, ensuring high standards of patient care and accurate documentation.
- Childhood & Adult Immunisations - Deliver vaccinations safely and effectively, following current immunisation schedules and maintaining up to date knowledge of vaccine protocols.
- Wound Care - Provide assessment, treatment, and ongoing management of acute and chronic wounds, including dressings, suture removal, and patient education.
- Support and manage patients with long term conditions such as asthma, COPD, diabetes, hypertension, and cardiovascular disease.
- Undertake annual reviews and personalised care planning.
- Use evidence based practice to deliver high quality care and promote patient self management.
- Work collaboratively with the wider clinical team to ensure continuity of care and effective person centred management.
- Maintain accurate, timely clinical records in accordance with practice policies.
- Participate in clinical audits, quality improvement initiatives, and multidisciplinary meetings.
- Contribute to the development of nursing services and practice protocols.
- Ensure infection control standards and safeguarding responsibilities are upheld.
- Provide compassionate, patient centred care at all times.

Person Specification
- Registered Nurse - NMC
- Experience in primary care or a strong interest in developing skills in this area.
- Competence in cervical screening, immunisations, and wound care or willingness to undertake training.
- Strong communication, organisational, and clinical decision making skills.
- Commitment to ongoing professional development.
- Driving licence - for home visits and to be able to work across both our sites - Woolston Lodge Surgery and Chartwell Green Surgery.
- Experience in chronic disease management clinics.
- Knowledge of SystmOne.
- Knowledge of Quality Framework Outcomes (QOF).
- Disclosure and Barring Service Check.
