The post holder will be a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. The post holder will work as part of a multi-disciplinary team. Responsibilities include:
- Take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
- Provide primary support to general practice staff with regards to prescription and medication queries.
- Support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing.
- Provide expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice(s).
- Provide clinical leadership on medicines optimisation and quality improvement.
- Manage some aspects of the quality and outcomes framework and enhanced services.
- Ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.
The role is pivotal to improving the quality of care and operational efficiencies, requiring motivation and passion to deliver excellent service within general practice.

MWEB PCN consists of 5 GP practices in Northamptonshire (Moulton, Woodview, Kingsthorpe, Wootton and Denton). Our PCN vision is to help all of our patients lead the best possible life from the cradle to the grave through a collaborative and team approach. We are a forward-thinking PCN with a constant view of how we evolve and are always looking for innovative and efficient approaches to our way of working. Our work models are a mixture of face-to-face and remote consultations. This is a PCN role specifically covering Moulton Surgery and Kingsthorpe Medical Centre with a combination of practice-based and remote working.

Person Specification
Knowledge & Skills Essential:
- Excellent interpersonal influencing and negotiating skills.
- Excellent written and verbal communication skills.
- Able to plan, manage, monitor, advise and review general medicine optimization issues in core areas for long-term conditions.
- Able to obtain and analyse complex technical information.
- Recognises priorities when problem-solving and identifies deviations from the normal pattern.
- Able to work under pressure and meet deadlines.
- Work effectively independently and as a team member.
- Demonstrates accountability for delivering professional expertise and direct service provision.
- Able to act upon MHRA alerts.
- Is a prescriber or working towards this competence.
Experience Essential:
- Min 2 to 4 years post-graduate experience (2 years in general practice or 4 years in other comparable healthcare settings).
- Experience of public health promotion/high-risk medicines monitoring/lead on antimicrobial stewardship/safe and effective repeat prescribing.
- Good working knowledge of QOF and contribution to QOF delivery.
- Good working knowledge of and role in Enhanced Services.
- An appreciation of the nature of GPs and general practices.
- Able to conduct clinical medicine reviews, reduce waste through poly-pharmacy and over-prescribing, reconcile and optimize medications.
Qualifications Essential:
- Mandatory registration with General Pharmaceutical Council.
- Membership of the Royal Pharmaceutical Society.
- Undergraduate Pharmacy Degree experience.
- Must be an Independent Prescriber or working towards this.
- Other post-graduate studies such as a PGDip.
