Role Overview:
Our Discharge Coordinators are an on-site presence providing discharge management support to patients and clinical teams in NHS Hospitals. You will play a key role in ilarna's operational patient discharge services on NHS Trust sites. You will champion a culture of excellence, ensuring that our service users are supported through their hospital discharge journey and that we meet our ambition of providing world-class care to all.
Key Responsibilities:
- Act as the main point of contact for discharge planning for eligible patients.
- Coordinate with the MDT, including doctors, nurses, therapists, and social workers, to identify and plan patient discharges from the point of admission.
- Liaise with patients and their families to explain discharge plans and expected timelines.
- Work closely with community services, ilarna's internal operations team, and social services to arrange appropriate care packages.
- Attend daily board rounds and discharge planning meetings.
- Maintain accurate records of discharge planning activities and patient pathways.
- Monitor and escalate delays or barriers to discharge, contributing to service improvements.
- Ensure that patients are discharged safely with all necessary medications, transport, and follow-up care arranged.
- Participate in audits and data collection to support performance monitoring and continuous improvement.
- Promote patient choice and independence wherever possible.
