Woodlands Park Health Centre seeks an experienced and motivated Practice Nurse to join our friendly and supportive clinical team. This role offers an exciting opportunity for a skilled nurse passionate about delivering high‑quality patient care, particularly in the management of long‑term conditions such as diabetes and respiratory disease, and includes key responsibilities in childhood immunisations, cervical screening and health promotion within our busy NHS GP practice.
Main duties of the job
- Lead and deliver high‑quality nursing care within a busy GP practice environment, with a focus on long‑term condition management (diabetes, asthma, COPD, hypertension, cardiovascular disease).
- Provide support in childhood immunisations, cervical screening, health promotion and work collaboratively with the multidisciplinary team.
Job responsibilities
- Deliver chronic disease management clinics, especially in diabetes, asthma, COPD, hypertension and cardiovascular disease, including annual reviews.
- Administer childhood and adult immunisations according to national guidance and local protocols.
- Carry out cervical cytology screening in line with the NHS Cervical Screening Programme.
- Provide health promotion, screening and NHS Health Checks, wound care, injections and general practice nursing duties.
- Maintain accurate and contemporaneous clinical records using SystmOne.
- Recognise, assess and appropriately raise safeguarding concerns or deteriorating patients, supporting practice in achieving QoF and contractual quality frameworks.
Person Specification – Experience
- Previous experience working within General Practice.
- Substantial experience managing long‑term conditions, particularly diabetes and respiratory disease.
- Experience undertaking cervical screening and childhood and adult immunisations.
- Experience working independently within a multidisciplinary clinical team.
- Experience delivering patient‑centred care within a busy clinical environment.
- Experience maintaining accurate clinical records and using clinical systems effectively.
- Desirable criteria include experience using SystmOne, supporting QoF and enhanced services, undertaking NHS Health Checks and health promotion activities, and mentoring or supporting staff or students.
Person Specification – Qualifications
- Registered Nurse with current NMC registration.
- Evidence of continuing professional development.
- Qualified and competent in cervical cytology screening.
- Qualified and competent in childhood and adult immunisations/vaccinations.
- Good understanding of infection prevention and control standards.
- Desirable criteria include experience using SystmOne, spirometry experience and previous involvement with QoF or enhanced services.
- Experience supporting students, mentoring colleagues or participating in service development and quality improvement initiatives would also be advantageous.
