Deliver care to residents on the unit by implementing the agreed care plan(s).
Communicate changes in the residents’ condition or care needs to the Team Leader and/or Nurse in Charge.
Report any concerns you have about the resident to the Team Leader or Nurse in Charge.
Involve the resident and, if appropriate, their relative in the implementation of the care plan.
Report illness, falls, and injury to the Nurse in Charge in a timely manner to ensure that residents’ health needs are effectively managed.
Display appropriate practices and attitudes in the provision of care to residents ensuring that their dignity and individualism is respected and that their confidentiality is maintained.
Adhere to company policies, local guidelines, and other directives.
Appropriately document your interventions to ensure that it is accurate, timely, legible, and appropriate to the care provided.
Work in a way to reduce the risks related to the control and management of infection.
Communicate effectively to enhance continuity and to reduce risk to residents.
Report any concern you may have about the care and welfare of the residents, including issues related to the safeguarding of residents.
