Woodcote Medical is delighted to offer an exciting opportunity for a dedicated and proactive PCN Paramedic to join our friendly multi-disciplinary team. This is a fantastic role for a paramedic who is passionate about high-quality patient care, minor ailments and collaborative working within general practice.

As part of the Purley & Coulsdon PCN, you will work closely with our GPs, nurses, ANPs, physiotherapists, and administrative teams to deliver safe, effective, and person-centred care across our population of approximately 18,000 patients. We are a supportive, teaching practice and take pride in helping clinicians develop, thrive, and build long-term careers in general practice.

Main duties of the job

As a PCN Paramedic, you will be a valued member of the clinical team, responsible for:

  • Telephone and face-to-face consultations for minor illness/injury
  • Clinical assessment, diagnosis, treatment, and referral
  • Community and care-home visits
  • Advising patients on health and wellbeing
  • Maintaining accurate clinical records
  • Supporting audits and service improvements
Operational & Collaborative Work

Work within own professional scope and code of conduct (HCPC and College of Paramedics). Supporting PCN quality and outcomes framework (QOF) and IIF targets. Contributing to audits, service improvement, and implementation of local and national guidelines.

Job responsibilities

The post holder aspires to provide a responsive, safe, effective and caring service to patients. Initial history taking, clinical and functional assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills, for patients within their own homes.

Along with using a medical model to assess, diagnose and prescribe for acute medical conditions, the clinician will use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.

The clinician will participate in, and influence efforts, across health and social care to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through proactive provision of care in or as close to the patient’s own home as possible.

Qualifications
  • Registered with HCPC
  • Experience as an autonomous practitioner within a minor injury/illness centre or mobile service.
  • Ability to work autonomously, using own clinical judgement to form a diagnosis
  • Ability to manage and prioritise own workload
  • Excellent communication skills and be able to function effectively as part of a team
  • A proactive, compassionate approach and a commitment to improving patient outcomes.
  • Pre-reg MSc or working towards ACP status
  • Primary care experience
  • Experience with EMIS

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

£45,000 to £50,000 a year dependent on experience

Contract: Permanent

Working pattern: Full-time, Part-time, Flexible working, Compressed hours
