Lead Care Navigator — Patient Experience & Team Lead in Nottingham

Wollaton Park Medical Centre in Nottingham is seeking a motivated Lead Care Navigator on a part-time basis to lead the Care Navigation Team and ensure effective service delivery across Monday to Friday.

You will coordinate patient care, signpost to services, maintain confidential records and support patients in navigating the health system with strong leadership and excellent communication.
