We have an excellent opportunity available for a Specialist Case Officer to join our CHC/117 Team in Adult Social Care here at Wokingham Borough Council, on a full time, permanent basis.

As the Specialist Case Officer, you will play a key role in identifying, receiving and logging potential cases for Continuing Health Care (CHC) and section 117 (S117), that are referred from colleagues or identified through assessment.

You will need to assess whether the cases would justify further investigation and determine what further action may be required.

Key Responsibilities

  • Processing referrals for CHC/117 clients
  • Collaborating with a multi-disciplinary team for CHC assessments
  • Attending MDT meetings
  • Collating and gathering evidence for clients with a Primary Health Need
  • Keeping accurate records of outcomes and decisions

Candidate Requirements

  • Recognised qualification in Social Work, Occupational Health or Nursing
  • Knowledge of the Care Act 2014
  • Knowledge of the NHS Continuing HealthCare Framework and the Section 117 process
  • Background in working with individuals with complex health needs
  • Full UK Driving License & use of car
