Woking & Sam Beare Hospice are pleased to announce the following position. We are a patient-centred charity that delivers palliative and end of life care to those with advanced life-limiting illnesses, who live in Northwest Surrey. We care for patients and provide support to families and carers in the community as well as in our In-Patient Unit and Well Being Centre at the Hospice. You will, therefore, be joining a passionate and dedicated team of professionals that make a real difference to the families across Surrey community.

Main duties of the job

Our therapy team consists of Occupational therapists, Physiotherapists, and Therapy assistants. We work closely with a team of Clinical Nurse Specialists, Palliative Care doctors, our Hospice at Home team, Counsellors and Complimentary therapists. The locations we see patients in are varied and this role includes working across inpatient, outpatient and community services. The ability to travel in your own car to visit patients at home is essential. This is an excellent opportunity to develop your clinical skills within a supportive and collaborative multidisciplinary environment. The successful candidate will provide physiotherapy assessments and treatments to patients with complex and life-limiting conditions, helping to maximise independence, manage symptoms and improve overall wellbeing.

  • Provide a high standard of individualised patient-centred care to patients with a progressing and life limiting illness.
  • You will need experience in a wide range of core Physiotherapy skills.
  • Work flexibly as a member of a speciality based team, under the guidance and supervision of the Senior Therapists.
  • Undertake skilled and effective communication with patients and their relatives as well as the wider multi-disciplinary team.
  • Competence in clinical reasoning and risk assessment.
  • Understanding of holistic and patient-centred care in palliative settings.
  • Excellent communication and interpersonal skills.

A successful candidate will have:

  • Degree in Physiotherapy.
  • Minimum of 3 years post registration.
  • Current registration with the Health and Care Professions Council (HCPC).
  • Evidence of Continuing Professional Development (CPD).
  • Experience of working as a physiotherapist within a multidisciplinary team (MDT).
  • Rotational experience across areas such as Neurology, Medical / Respiratory, Community.

Job responsibilities

To provide a high standard of individualised patient-centred care to patients with a progressing and life limiting illness. You will need experience in a wide range of core Physiotherapy skills. This job will require the job holder to work flexibly as a member of a speciality based team, under the guidance and supervision of the Senior Therapists. The post holder will be required to undertake skilled and effective communication with patients and their relatives as well as the wider multi-disciplinary team. The post holder will work flexibly across community, inpatient and wellbeing services offered to patients of Woking and Sam Beare Hospice.

Person Specification

  • Car driver with a full driving license.
  • Have access to a vehicle for community visits and ensure appropriate insurance in place.
  • Excellent communication and interpersonal skills with patients, families and professionals.
  • Ability to cope with sensitive and emotionally challenging situations.
  • Ability to empathise with patients that are terminally ill.
  • Good organisational and time management skills.
  • Teaching or mentoring skills.
  • Experience facilitating group sessions or patient education.

Qualifications

  • Degree in Physiotherapy.
  • Minimum of 3 years post registration.
  • Current registration with the Health and Care Professions Council (HCPC).
  • Evidence of Continuing Professional Development (CPD).
  • Evidence of working within MDTs dealing with complex neurological, respiratory and cancer cases.
  • Postgraduate training or modules in palliative care, oncology or related areas.
  • Membership of a specialist group such as the Association of Chartered Physiotherapists in Oncology and Palliative Care.

Experience

  • Experience of working as a physiotherapist within a multidisciplinary team (MDT).
  • Rotational experience across areas such as Neurology, Medical / Respiratory, Community.
  • Knowledge of evidence-based physiotherapy practice.
  • Able to demonstrate awareness of current issues relevant to physiotherapy practice.
  • Ability to anticipate disease progression and adapt rehabilitation goals.
  • Skills in symptom management and functional rehabilitation.
  • Competence in clinical reasoning and risk assessment.
  • Understanding of holistic and patient-centred care in palliative settings.
  • Awareness of resource management issues.
  • Excellent clinical knowledge and skills.
  • Evidence of specialist skills for clinical practice in symptom control or palliative rehabilitation.
  • Post graduate courses.
  • Knowledge of approaches for treatment for patients with a terminal disease.
  • Advanced IT Skills.
  • Knowledge of EMIS.
