We are looking for a dedicated Health Care Assistant to join our Hospice at Home team, providing high-quality palliative and end-of-life care to patients wherever they call home. This is a rewarding opportunity to support people at an important time in their lives, helping them receive safe, personalised and compassionate care in their preferred place of care.
Main duties of the role
- Working under the supervision of Staff Nurses and Specialist Practitioners, you will deliver person-centred care and support to patients, families and carers.
- This will include assisting with personal care, supporting symptom management, observing and reporting changes in condition, maintaining accurate records and communicating sensitively with those receiving care and the wider multidisciplinary team.
About you
- You will have experience of providing direct patient care in a health or social care setting, along with excellent communication skills and a compassionate, professional approach.
- An NVQ Level 3 in Health and Social Care, or a willingness to work towards this, is essential.
- Experience in palliative, end-of-life, community or hospice care would be advantageous.
- You must have the legal right to work in the UK and be willing to undertake the relevant DBS check.
- You will also need to be able to meet the practical requirements of the role, including holding a full UK driving licence and having access to a vehicle for work purposes.
