About the Role

The Urgent Care Team provides a 7‑day service to support complex discharges in the acute setting and community. The role may involve working weekends and bank holidays, with additional pay for those days.

Key Responsibilities

  • Manage your own caseload to support complex hospital discharges.
  • Identify adults at risk of abuse or neglect, and complete MCA assessments.
  • Work within a multidisciplinary environment, utilizing the 3 Conversation model and the Care Act 2014.
  • Support patients to establish goals and work towards returning to home, applying the Mental Capacity Act 2005.
  • Provide regular supervision and reflection to develop knowledge, skills, and practice.

Qualifications & Experience

Relevant qualifications and experience in social care, mental health or related fields are required.
