Our approach is to offer short-term intensive support to enable individuals to learn how to live their lives as independently as they can and support them to regain independence during the period of support. We will continue to focus on what people can do and build on these skills. We will make sure people receive the right support at the right time to help them live independently at home for as long as possible. This is part of a broader approach to ensure Worcestershire residents are healthier, live longer, have better quality of life and remain independent for as long as possible.

Responsibilities

  • Undertake initial assessments within the community; identifying needs, risks and the service user's potential for regaining independence with personal and/or domestic activities of daily living.
  • Develop Promoting Independence Plans that capture the desired service user outcomes and detail the required intervention of the service.
  • Actively case manage service users, as part of a multidisciplinary team, within a locality including regular reviews of their progress.
  • Ensure that all standards and performance measures are adhered to, including service user monitoring protocols and quality assurance audit completion.
  • Provide out of hours cover as part of a rota during hours of service delivery.

Qualifications

As the ideal candidate you must have a level 3 Diploma in Health and Social Care or considerable experience of working in a social care setting. It is also equally important that you can demonstrate a can-do attitude, and that you are proactive, compassionate and empathetic.
