Join us at this exciting time as a Hospital Social Worker within our citywide Hospital Social Work team. You’ll play a key role in supporting timely discharges from all hospital settings.
Responsibilities
- Support people leaving hospital or discharge to assess services, working closely with carers and support networks.
- Lead and influence safe, timely discharges as part of a multidisciplinary team.
- Understand funding routes and relevant legislation (Care Act, MCA, discharge policy).
- Produce clear reports, assessments and plans, and monitor positive outcomes.
- Champion strengths-based practice and ensure individuals’ voices are central.
- Work collaboratively with partners including health, police, voluntary sector and providers.
About You
- Be a registered Social Worker (SWE).
- Have completed your ASYE or equivalent.
- Hospital experience will be desirable.
- Have strong analytical, assessment, organisational and communication skills.
- Be committed to professional development and high-quality practice.
- An Enhanced DBS is required.

All council roles are allocated to either a fixed, field or flexible work style. Some roles are agile which enable and empower employees to maximise their performance and productivity, whilst maintaining a healthy work life balance. Roles with a field workstyle are usually allocated where employees are required to be out working in the community with a minimum requirement for touch down space at any council location. Roles with a flexible workstyle have a flexible base location to meet the needs of the business and employees can work from a mixture of home or any council office, partner/client, or external location, as required to meet service requirements.
