About the Role

This is an exciting opportunity to join our newly re‑structured Disabilities and Mental Health Service. You will carry out conversations to keep in touch (review) with people who have care and support plans in place. The role supports individuals in living their ideal life and connecting to community.

Key Responsibilities

  • Use creativity to ensure people with disabilities and mental health needs are supported to live gloriously ordinary lives and connected to their communities.
  • Ensure people’s voices are heard and represented in care and support plans.
  • Support young adults navigating early adulthood with confidence.
  • Work with people at risk of or involved with the criminal justice system to build a good life based on care and support plans that promote safety and independence.

Requirements

  • Dedicated to working with people with disabilities and mental health needs and their carers/families.
  • Sound working knowledge of relevant legislation.
  • Strong analytical, assessment and negotiation skills.
  • Passionate about strengths‑based working.
  • Ability to work with colleagues, service providers, partners and commissioners.
  • Strong interpersonal, communication and IT skills.
  • Registered with Social Work England.
  • Approved Mental Health Professional/Best Interests Assessor or willing to undertake training.
  • Vision for own development.
  • Completed the Assessed and Supported Year in Employment (ASYE) or an equivalent post‑qualifying pathway.
