The Role:
- Play your part in ensuring that the team is effective and efficient in delivering its core purpose to the citizens of Stoke-on-Trent.
- Ensure that people are fully involved and empowered throughout their contact with the service.
- Ensure that the person is fully understood in order that the most appropriate tailored solutions can be offered.
- Identify and address any system barriers which prevent people from experiencing the most efficient and effective journey through the service.
- Ensure that people are empowered, connected, independent and have an improved sense of well-being through the provision of outcome-focused support.
- Meet with people regularly to discuss their experiences of the service to drive further improvement.
Key Responsibilities:
- Social Care assessments for adults and carers.
- Reviews, care and support planning, and continuing health care checklist and decision support tools.
About You:
- Requirement to accurately maintain client records and ensure that these records are logged correctly on the department's IT system.
- Strong report writing and communication skills.
- Relevant knowledge and experience in Adult Social Care either in a formal or informal capacity.
- Able to fully meet the requirements of the post and cover the whole geographic area of the City either by possession of a current and valid driving licence or by access to public or other transport.
