About The Role

We are looking for an experienced Social Worker to join our expanding Social Care and Support Service. The post is based within the Older People's Team, specialising in Continuing Health Care (CHC). The role focuses on undertaking Decision Support Tools (DSTs), contributing to multidisciplinary team processes, and providing specialist advice and guidance on CHC to community-based social work teams to support consistent, high-quality CHC practice.
