Warwickshire County Council (WCC) is seeking a Duty Coordinator to support us in offering high quality customer service within the Duty Team of the Community Health Discharge Team.
Social Care and Support – Warwickshire County Council works within communities to build strong relationships and connections. We focus on prevention and maximise positive outcomes by using strength‑based, community‑led support approaches. - support the duty team to provide high quality customer service within the Community Health Discharge Team
- contribute to ensuring effective, person‑centred practices that start with strengths
- work collaboratively with partners to enable positive outcomes for adults
