Warwickshire County Council is seeking a Social Worker to join the Discharge to Assess team.
You will provide a high-standard social work service, manage a caseload, and travel across North and South Warwickshire to support timely discharges.
You will attend Continuing Health Care / Decision Support Tool (DSTs) meetings and record robust assessments and care planning.
You will work with adults, their families and unpaid carers, often in pressurised situations.
