Community Link Worker (Youth) in Telford

Hours: 12 hours per week. (We have two 12 hours per week vacancies).

Job Purpose

• Provide a single point of contact and holistic community-based solutions for individuals with a specific focus on supporting Young Carers.
• Raise awareness and advocate for Unpaid Family Carers, promoting greater understanding and support for those with caring responsibilities.
• Offer information, advice, assisted access to services, practical support in community settings and during respite activities.
• Use innovative and creative approaches to support people to live well, closer to home.

Main Duties and Responsibilities

Community Response & Engagement

• Respond to referrals and self-referrals into the All Age Carers Centre from both the public and professionals.
• Provide advice, information, support, and signposting for Unpaid Family Carers.
• Respond to queries via telephone helpline and email inbox.
• Contribute content to newsletters, websites, and other communication channels.
• Engage with members of the public at community hubs, meetings and youth clubs, offering practical, community-based solutions.
• Engaging with Young Carers outside of school hours to provide activities during evenings, weekends and school holidays, therefore this role will work outside of our normal office hours to meet the needs of the service.

Carer Support

• Deliver creative activities, group sessions, and 1:1 drop-ins (across the community).
• Support Young Carers in balancing their caring role with their own personal development, education employment and wellbeing.
• Provide outcome-focused casework for Young Carers, ensuring needs are addressed holistically.
• Build strong, trusting relationships with individuals of all ages, backgrounds, and circumstances.
• Support the individual on their caring journey – particularly with transitioning into adulthood.

Casework & Multiagency Working

• Work in partnership with internal colleagues, statutory services, and voluntary partners to provide joined-up support.
• Support and motivate individuals to develop their own solutions to improve health and wellbeing.

Monitoring & Evaluation

• Maintain accurate records of all support provided, in line with organisational policies.
• Manage data and produce regular reports to evidence outcomes.
• Monitor and evaluate cases, including measuring impact and gathering feedback on satisfaction.
• Collating feedback and views to inform future provision including case studies, quarterly report and presenting information to the Carers Partnership Board.

Outreach & Awareness

• Represent the service at local events and community organisations.
• Deliver presentations and workshops to professionals and organisations, promoting the identification and activity provision available to Young Carers in the local area.
• Actively raise community awareness and engage with Young Carers, ensuring support is accessible and visible through a variety of mediums including professional social media posts, website updates, flyer design and the quarterly magazine.
• Generate income and donations to support Young Carer activity provision.
• Build relationships and networks to improve the Young Carer local offer.

The successful applicant may from time to time be asked to carry out other duties within the organisation in line with the requirements of the CVS.

Telford & Wrekin CVS operates a friendly, informal team. However, we aim to maintain the highest standards of service and efficiency. Experience of working in a small team in a busy office environment would, therefore, be an advantage.

Support, supervision and training, where necessary, will be provided.

Qualifications / Experience

Evidence of a solid general education is required together with GCSE A*-C / 9-4 or equivalent including English and Mathematics. Developed keyboard skills and the ability to use Microsoft Office applications, i.e. Word, Excel, Outlook, Internet Explorer and an understanding or knowledge of Access are also essential.

Have or be working towards a Level 3 Youth Work qualification.

A relevant professional qualification NVQ Level 3 Health and Social Care, or equivalent.

Evidence of further education or training that would support your application.

Hours of Work

Normal office hours:

Monday - Thursday: 9.00 am - 5.00 pm, Friday: 9.00am - 4.00pm

“Out of Hours” work and a flexible working pattern will be necessary to include working evenings and weekends.

Time off in lieu can be arranged (overtime is not paid).

The postholder will be required to undertake an Enhanced Disclosure and Barring Service check.
